Notice to our Patients
Moab Valley Healthcare, Inc., is committed to protecting the security and confidentiality of our patients’ information. Regrettably, we are writing to inform you about an incident involving some of that information.
On August 19, 2019, we learned that following the closing of the old Allen Memorial Hospital facility in February 2011, and a 2017 re-sweep of the old building for any documents, some patient information was nonetheless left at the facility. We immediately began an investigation and determined that some paper laboratory results contained in folders used for laboratory equipment calibration had remained in the old facility. These records included patient names, addresses, dates of birth, phone numbers, medical record numbers, types of studies, dates of service, laboratory values and results. Patients’ Social Security numbers and financial information were not contained in these records.
This incident does not affect all Moab patients; only a small number of patients that received laboratory services prior to 2008 at Allen Memorial Hospital.
We have no indication that any patient information has been actually accessed or viewed by unauthorized persons. However, in an abundance of caution, we mailed letters to affected patients on October 18, 2019, and established a dedicated call center to answer questions patients may have. If you believe you have been affected and do not receive a letter by November 18, 2019, please call 1-844-996-1032, Monday through Friday, 7:00 a.m. to 4:30 p.m. Mountain Time.
We deeply regret any concern this incident may cause you. To help prevent something like this from happening in the future, we have retrieved all Moab Valley Healthcare documents that remained in the facility.
